PAYMENTS
Payments are due at the time of service. Any labs, referrals for imaging, certain medications, or testing may be covered by your insurance. It is your responsibility to know what your insurance covers. We do accept payments from HSA or FSA accounts.
CANCELLATION POLICY
We require a 48-hour cancellation notice. If you cancel your appointment with less than 48 hours’ notice or fail to show for your appointment without notification you will be charged $25.
CONFIDENTIALITY STATEMENT
Your privacy is important to us. All medical records and interactions between providers and patients are completely confidential. You will be provided and asked to sign a confidentiality statement when you establish care with our office.
COVID-19 POLICIES
Our policies state that if you are displaying symptoms of an acute illness such as fever, cough, body aches, loss of taste or smell, please notify the office ahead of your visit. A telehealth appointment may be arranged if deemed appropriate. If displaying any acute illness symptoms, please wear a mask when entering the office.
